Hospice Community Liaison Location: North Miami, Miami Beach, Aventura Florida Industry: Hospice / Healthcare Experience Level: Mid-Senior (Healthcare Required) Position Overview OpusCare is seeking an experienced Hospice Community Liaison with a proven background in hospice, healthcare, and patient relations. This role is critical to building trusted relationships with referral sources, supporting patients and families through the hospice decision process, and representing OpusCare with professionalism, empathy, and clinical understanding. This position is not suitable for candidates without prior healthcare, hospice, or patient-facing experience. Required Experience & Qualifications (Must Have) Applicants must meet all requirements below to be considered: * Minimum of 2 years of experience in hospice, home health, healthcare sales, or patient relations * Demonstrated experience working directly with patients, families, physicians, case managers, social workers, or discharge planners * Strong understanding of: * Hospice philosophy, services, and eligibility criteria * Patient and family dynamics during serious illness and end-of-life care * Proven ability to build clinical and referral relationships within hospitals, physician offices, SNFs, ALFs, or community healthcare settings * Excellent interpersonal, communication, and presentation skills * Ability to navigate sensitive conversations with compassion, professionalism, and confidence Applicants without healthcare or patient-facing experience will not be considered. Key Responsibilities * Develop and maintain strong relationships with healthcare providers, referral sources, and community partners * Educate referral partners on hospice services, eligibility, and care coordination * Serve as a trusted liaison between OpusCare, patients, families, and referral sources * Support patient admissions by collaborating with clinical teams to ensure smooth transitions of care * Conduct in-services, presentations, and educational outreach within healthcare settings * Manage assigned territory and referral accounts with accountability and professionalism * Accurately track referral activity and community engagement efforts * Maintain compliance with hospice regulations, HIPAA, and company policies Preferred Experience (Highly Valued) * Prior Hospice Liaison, Community Liaison, Marketing Representative, or Admissions experience * Background as a social worker, nurse, CNA, medical assistant, or healthcare coordinator * Established referral relationships within the local healthcare community Why This Matters This role requires more than sales skills -- it requires clinical awareness, emotional intelligence, and a deep respect for patients and families navigating end-of-life care.
