The Opportunity We are currently seeking a dedicated Nursing Home Hospital Liaison What You Will Do * Build and maintain strong relationships with hospital case managers, social workers, discharge planners, and physicians * Work closely with our centralized review department to execute a bedside visit with potential admissions candidates (PAC) * Present diagnosis specific marketing collaterals to potential nursing home admission candidates and influencers * Coordinate facility tours and interaction between PAC/influencer and facility specific staff * Collaborate with facility admission teams to ensure efficient and timely admission processes * Identify and overcome barriers to admission, including clinical, insurance, and logistical concerns * Provide a full time presence in designated hospitals * Inform hospital staff, PAC and influencers on Harborview's clinical capabilities and differentiation between levels of care * Duties as assigned What You Must Have * Active RN/LPN license (or related) in good standing in the state of practice. * Experience in a skilled nursing or long-term care setting is preferred but not required--we'll invest in the right heart and mindset.
