Job Overview We are seeking a passionate and dedicated Community Representative to join our team. This role is pivotal in fostering relationships within the community, promoting our programs, and ensuring effective communication between stakeholders. The ideal candidate will possess strong leadership qualities and a commitment to public health and social work, with experience in volunteer management and program development. Responsibilities * Develop and maintain relationships with various organizations * Responsible for Submitting Daily Reports to Management. * Building relationships, receiving referrals and promoting our agency with * Hospitals, Nursing Homes community organizations and physician practices. * Managing and creating new referral sources (nursing homes, hospitals, * community referrals etc.) * Conduct presentations to support groups, adult day cares, Community Events * as well as conduct home visits, and personalized appointments as needed to * complete the enrollment process. * Follow up on leads and referrals from family members, caregivers, and other * sources. * Engage in public speaking events to promote initiatives and raise awareness about home care services * Collect and analyze data to evaluate program effectiveness and inform future strategies. * Utilize social media platforms for marketing efforts, enhancing community engagement and outreach. * Collaborate with local organizations for fundraising efforts and community events. * Conduct training sessions for volunteers and community members on relevant topics related to public health and social services. Qualifications * Strong public speaking skills with the ability to engage diverse audiences effectively. * Background in program development, project management, or marketing preferred. * Experience in social media management to enhance outreach initiatives. * Demonstrated leadership abilities with a passion for community service and advocacy. * Self-motivated * Must have a car * bi-lingual- English/Spanish/Chinese/Creole Job Type: Full-time Pay: $47,957.17 - $200,000.00 per year Benefits: * Dental insurance * Flexible schedule * Health insurance * Paid time off Work Location: On the road
