Job Title: Hospice Community Liaison & Social Worker (Dual Role) Company: Superior Hospice Compensation: $65,000 -$85,000 per year (DOE) + Sales Commission! Superior Hospice is looking for a people-forward, outgoing Social Worker who thrives both in the field with patients and out in the community building relationships. This is a unique dual-role position combining clinical social work visits with hospice outreach and sales efforts across long-term care and senior living communities. This role is ideal for someone who can connect quickly , tell our story confidently , and represent hospice with heart and professionalism. You'll support patients and families directly while also serving as a trusted, familiar face to referral partners. What You 'll Do * Complete social work visits with hospice patients and families, providing emotional support, education, and care coordination * Participate in interdisciplinary team meetings and collaborate with clinical staff * Build and maintain strong relationships with long-term care, assisted living, and senior housing communities * Conduct in-person outreach to Administrators, Directors of Nursing, Social Services, and leadership teams * Educate referral partners on hospice services, eligibility, and Superior Hospice 's approach to care * Attend community events, meetings, and in-services as a representative of Superior Hospice * Split time between clinical visits and outreach efforts based on organizational and census needs Who Thrives in This Role * Licensed Social Worker in Minnesota (LSW or LICSW) * Outgoing, confident, and comfortable initiating conversations and building rapport * Passionate about hospice care and able to sell themselves and the organization authentically * Strong communicator who can speak comfortably with families, clinicians, and facility leadership * Self-motivated, organized, and able to manage a dynamic, blended role * Reliable transportation and comfort traveling between communities Why This Role Is Different * A blended clinical and outreach role that keeps every week varied * Opportunity to make an impact through both patient care and community relationships * Full-time stability with competitive salary * Supportive hospice team that values personality, initiative, and compassion * Ability to grow professionally in both hospice social work and community engagement If you're a Social Worker who loves connecting with people, believes in hospice, and wants a role that blends heart, hustle, and purpose , this position offers a meaningful and energizing career path.
