At Legacy Home Care , we're expanding--and we're looking for a proven prospector to identify, develop, and convert new business across the Twin Cities metro. As a Community Liaison , you'll be the face of our services in the field-- educating discharge planners, social workers, and providers while creating warm handoffs for families in need of home health support. You'll have the autonomy to own your territory , and the backing of a well-established, mission-driven team.
