What You 'll Do * Conduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical Manager * Develop, implement, and update individualized plans of care based on patient needs and physician orders * Perform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home setting * Complete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as needed * Complete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicable * Observe, document, and report patient response, progress, and any significant changes to the physician/provider and care team * Communicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality care * Provide education and support to patients and their families to promote safety, independence, and long-term success * Assist with orientation, supervision, and consultation for Physical Therapy Assistants, as assigned Who You Are * Compassionate, dependable, and patient-focused * Comfortable working independently in the home setting * A collaborative team player who values communication and support * Motivated by meaningful, one-on-one patient care Qualifications * Master's degree in Physical Therapy * Active Physical Therapy license in good standing in the state of California * At least one year of experience working as a Physical Therapist * Must have a vehicle, a valid current driver's license, and automobile insurance * Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and * submit in a timely manner * Home Health and Home Care Home Base documentation experience Schedule Options We are currently hiring for Full-Time and Per Diem Physical Therapy opportunities.
