About Vita Blue Home Care: Vita Blue Home Care provides high-quality, client-focused home care services throughout the region. We are seeking an experienced and results-driven home care sales professional who understands referral development and can generate measurable growth through strong community relationships. Position Overview: This is a salary-based hybrid role. The Community Liaison is expected to: * Spend most of the week in the field meeting referral partners, conducting outreach, and generating new business , and * Be in the office at least two days per week for reporting, strategy development, and coordination with internal teams Required Qualifications: * Proven track record in home care sales is required * Experience cultivating relationships with healthcare referral sources (SNFs, hospitals, rehabilitation centers, social workers, etc.) * Ability to independently manage territory planning, scheduling, and follow-up * Strong communication, presentation, and closing skills * Valid driver's license and reliable transportation * Bilingual preferred Key Responsibilities: * Develop and strengthen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and community organizations * Conduct regular in-person visits and professional presentations * Generate qualified leads and convert referrals into active clients * Maintain detailed activity logs and reporting * Work closely with internal teams to ensure strong client onboarding and service continuity * Meet monthly and quarterly sales goals Compensation & Benefits: * $55,000 - $72,599 base salary * $500 commission for each new client who remains active for 30 days * Additional performance incentives * Health, dental, and vision benefits * Paid time off and mileage reimbursement * Supportive leadership and opportunities for professional advancement Why Vita Blue Home Care: This role is ideal for someone who is competitive, self-directed, and deeply experienced in the home care market.
