About Vita Blue Home Care: Vita Blue Home Care provides high-quality, client-focused home care services throughout the region. We are seeking an experienced and results-driven home care sales professional who understands referral development and can generate measurable growth through strong community relationships. Position

Overview

: This is a salary-based hybrid role. The Community Liaison is expected to:

• Spend most of the week in the field meeting referral partners, conducting outreach, and generating new business , and
• Be in the office at least two days per week for reporting, strategy development, and coordination with internal teams

Required Qualifications

:

• Proven track record in home care sales is required
• Experience cultivating relationships with healthcare referral sources (SNFs, hospitals, rehabilitation centers, social workers, etc.)
• Ability to independently manage territory planning, scheduling, and follow-up
• Strong communication, presentation, and closing skills
• Valid driver's license and reliable transportation
• Bilingual preferred

Key Responsibilities

:

• Develop and strengthen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and community organizations
• Conduct regular in-person visits and professional presentations
• Generate qualified leads and convert referrals into active clients
• Maintain detailed activity logs and reporting
• Work closely with internal teams to ensure strong client onboarding and service continuity
• Meet monthly and quarterly sales goals

• $55,000 - $72,599 base salary
• $500 commission for each new client who remains active for 30 days
