Essential Functions
The primary responsibility of the Community Liaison (Hospice) is to facilitate a seamless transition for patients discharging from a facility setting to the care of CompleteOK for post-acute care needs. The LIAISON role is aligned with company market development initiatives. This individual understands their role in growth, serving more patients and delivering exceptional care.
- Explains hospice services and agency procedures to the patient and his/her family members
- Involves the family|caregivers in the educational process and assesses post-discharge educational|coaching needs
- Knowledgeable about state specific admission criteria and timelines for admission
- Monitors the status of all patients receiving Respite or General Inpatient Care and facilitate the communication between the agency|hospital|physician
- Develop and maintain ongoing relationships with referral sources
- Become the subject matter expert on the Medicare Hospice Benefit
- Educate patients, family members, referral sources on the benefits of the hospice plan of care
- Assist CompleteOK Intake department with obtaining all information needed for patient admission
- Educate, complete legal documents with patient, family or POA for admission to services
- Communicate efficiently and effectively with patient, referral source, family, hospice clinical team, and Medical Director when appropriate
- Meet and exceed admission goals outlined by the VP of Hospice Sales
- Coordinate and host community and marketplace events, and educational lunches Experience
Requirements
One year of hospice sales/marketing experience License
Requirements
- Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation. Preferred Skill
Requirements
- Healthcare Marketing Experience
- Experience Managing Healthcare Accounts
- Proficiency with Microsoft Office (Excel, PowerPoint, Word)
- Sales Experience Preferred
