Job Description The Hospice Liaison serves as the primary community representative for the organization, responsible for generating qualified referrals through authentic relationship-building with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare partners. This role combines healthcare marketing, outreach, and clinical insight to communicate hospice services effectively and ensure timely access to care. Working closely with clinical and admissions teams, the Liaison supports seamless patient transitions and promotes Journey's mission of compassionate, high-quality service. . Education & Work Experience Requirements * 2+ years of experience as a Hospice Liaison, Healthcare Marketer, or Business Development Representative * Established referral relationships within SNFs, ALFs, hospitals, home health, or primary care networks * Strong understanding of hospice eligibility, services, and end-of-life care dynamics .
