As a Clinical Liaison, you will serve as the primary point of contact between new and existing patients of Southeast and area psychiatric hospitals. You will facilitate continued treatment to clients of Southeast while they are hospitalized at private psychiatric hospitals, as well as assessing new clients for admission to our community treatment teams. Key Responsibilities: * Meet with patients in the hospital setting to complete intake assessment and team assignment. * Advocate on behalf of clients to facilitate collaborative discharge planning between the hospital social worker and the agency case manager. * Facilitate the initial linkage meeting between the client and case manager while the client is still in the hospital. * Build and maintain positive relationships with hospitals, clinics, and other medical providers to ensure collaborative care.
