This role operates in a fast-paced, technology-driven environment and contributes to sales achievement for maximum profitability and growth aligned with the company's vision and values. The role also involves collaborating with leadership to refine sales strategies and use data to identify new market opportunities. Effective communication and collaboration with the Clinical and Admissions departments are essential for success. Stewardship * Serve as the primary point of contact during care transitions to ensure a smooth and effective process. * Maintain accurate demographic and medical records, verifying insurance eligibility. * Facilitate the discharge process and ensure all documentation and approvals are in place. * Educate patients and families about home healthcare services and expectations. * Develop and execute a business plan to achieve company sales goals and profitability. * Maintain relationships with established clients and generate new referrals for home healthcare services. * Identify opportunities to enhance partnerships with referral sources. * Act as a brand ambassador by representing the company in community outreach events, healthcare conferences, and professional forums. Customer Experience * Build and maintain relationships with patients, families, and referral sources to promote seamless care transitions. * Ensure continuity of care, reduce readmission rates, and enhance patient satisfaction. * Implement feedback mechanisms to improve the customer experience and address any challenges promptly. Technical Performance and Precision * Ensure documentation, including medical records, authorizations, and care plans, are accurate and timely. * Stay informed about regulations and policies related to care transitions, patient safety, and home health services. * Leverage CRM system to track sales activity, monitor referral sources, and manage the client pipeline. Learning and Professional Growth * Commit to continuous learning and professional development by attending sales training, healthcare seminars, and technology workshops. * Stay updated on industry trends, competitor analysis, and emerging home healthcare practices. * Participate in company-provided training to ensure knowledge of company offerings and compliance standards. Key Performance Indicators (KPIs) * Sales Targets: Meet or exceed monthly and quarterly sales quotas. * Referral Growth: Increase the number of referrals from healthcare providers and community partners by 10% year-over-year. * Conversion Rates: Maintain a referral-to-admission conversion rate of at least 80%. * Client Retention: Achieve a 90% or higher retention rate within the first six months of care. * Documentation Compliance: Ensure 100% accuracy in documentation and timely submission within 24 hours of transition. * Customer Satisfaction: Maintain patient and family satisfaction scores above 4.5/5.0 as measured by surveys and feedback. * Outreach Effectiveness: Conduct at least 20 outreach calls or visits weekly to new and existing referral sources. * Community Engagement: Participate in at least two community or industry events per quarter to promote company services. Qualifications * 5-7 years of sales experience in healthcare, with a preference for home healthcare. * Bachelor's degree in nursing, business administration, marketing, sales, or a related field (equivalent work experience may be considered). * In-depth knowledge of Medicare, Medicaid, and private insurance for home health services. * Experience working with CRM tools and data analytics for sales strategy. * Demonstrated ability to meet and exceed sales goals. * Willingness to travel locally to build and maintain referral networks. Technology * Proficiency in CRM software and data analytics tools. * Advanced use of MS Office Suite (Word, Excel, PowerPoint, Outlook) and Adobe. * Ability to use virtual communication platforms for remote collaboration. * Must have a private and secure home office space if occasionally working remotely. Travel Regular local travel is required to meet with healthcare providers, attend community events, and manage client relationships.
