We're looking for a dedicated Clinical Liaison to join our growing healthcare team and make a meaningful impact on the lives of patients and their families as well as yourself! WHAT YOU WILL DO: * Serve as the primary link between healthcare providers and our care services. * Build and maintain strong relationships with case managers, discharge planners, resident care directors, social workers, physicians, hospitals, and other referral sources you may have. * Assess potential patients' clinical needs and communicate those effectively to our care team. * Educate referrers and their patients about our services and coordinate smooth transitions to our care team.
