Community Liaison - Home Health Care Presidential HealthCare at Home LLC - Greater Boston & South Shore Job Type: Flexible Part-Time | Field-Based Compensation Structure: * Hourly Base Rate: $35-$45/hour depending on experience * Uncapped Commission: Based on census growth and referrals * Performance Bonuses: Eligible for additional incentive pay based on meeting growth targets Schedule : Flexible, 20-24 hours/week with on-the-road travel across assigned territory. The Opportunity We are hiring an experienced Community Liaison to lead our outreach and referral growth strategy. You will be the face of our agency--cultivating partnerships, presenting our services, and driving referral volume. Key Responsibilities - Develop and manage referral relationships with hospitals, rehab centers, and physicians - Identify new opportunities and expand our service territory - Promote agency services to discharge planners and case managers - Collaborate with leadership to meet monthly growth targets - Monitor and analyze referral trends to refine outreach strategies What We 're Looking For - Minimum 2 year of experience in healthcare marketing, business development, or discharge planning - Proven success in growing home health, hospice, or SNF census - Strong communication and relationship-building skills - Organized, self-motivated, and results-driven - Valid driver's license and ability to travel within the assigned region What We Offer - Competitive hourly pay plus uncapped earnings - Bonus structure directly tied to census growth -Ongoing support - A respected, mission-driven team with deep community ties How to Apply Apply on Indeed or visit [please apply online] Interviews are being scheduled now.
