Job Description: Responsible for building rapport with healthcare professionals. Educating community. Responsible for interacting and assisting with referrals from healthcare professionals in hospitals, home health agencies, skilled nursing facilities, long term care facilities and physicians. Responsible to educate patients/families on the benefits of hospice, work closely with HCC to assure a smooth admission process is followed and the referral source is informed during the process. Essential Functions: * Assesses and provides patient and family/ caregiver education and information pertinent to diagnosis as related to Hospice qualifiers * Serves as a educational resource to the health care professionals and community at large, regarding healthcare services. Coordinates available resource for staff and families. * Develops effective rapport with physicians, community associates, discharge planners and other referral sources and regularly visits them. * Serves as a liaison between HCC and physicians, hospitals, long-erm care facilities, assisted living facilities and all other referral sources to identify healthcare needs and assist with meeting those through Red Rock Hospice. * Serves as Hospice representative in the community to promote a positive image of Hospice and promote interest in services. * Works closely with HCC, intake, CSM, DCS to coordinate necessary services for patients and to promote communication between staff the referral source. Requirement * Must have LVN License * Clinical or case management experience * Strong problem solving, time management, and critical thinking skills.
