Overview: The Hospice Business Development Liaison is responsible for facilitating and fostering relationships with healthcare providers, hospitals, and community organizations to promote the hospice program and services and drive admissions. The liaison will serve as a key point of contact for referrals, educate healthcare professionals and families about hospice care, and ensure a seamless transition of patients into the hospice care. This role requires a compassionate and knowledgeable individual who can effectively communicate the benefits of hospice care and ensure the highest quality of service for patients and their families. Key Responsibilities: Referral Development & Management: * Develop and maintain relationships with local healthcare providers, including physicians, hospitals, nursing homes, and community organizations to generate patient referrals. * Respond promptly to incoming referrals and assess patient eligibility for hospice services. * Collaborate with the admissions team to ensure a smooth and timely transition of patients to hospice care. Community Outreach & Education: * Educate healthcare professionals, families, and the community about hospice services, end-of-life care, and the hospice philosophy. * Conduct in-service presentations, workshops, and training sessions for healthcare staff and community groups. * Act as an ambassador for hospice care, advocating for patients and families in various healthcare settings. Patient & Family Support: * Provide emotional and informational support to patients and families as they consider hospice care. * Assist families in understanding the benefits of hospice care, the services provided, and the process of enrollment. * Ensure that all inquiries and concerns from patients and families are addressed in a timely and compassionate manner. Collaboration with Internal Teams : * Work closely with the hospice clinical team, including nurses, social workers, chaplains, and physicians, to ensure that patient needs are met. * Participate in team meetings and contribute to the care planning process for referred patients. Compliance & Documentation: * Ensure all necessary documentation is completed accurately and in a timely manner. * Adhere to all federal, state, and local regulations related to hospice care, including HIPAA and Medicare guidelines. * Maintain confidentiality and privacy of patient information. Qualifications: * Bachelor's , sales and marketing, communications, business or a related field Preferred but not required. In lieu of a degree equivalent work experience will be considered. * Experience: In sales, road warrior, closing * Minimum of 2 years of experience in hospice care, home health care, healthcare sales, or a related field. * Experience in community outreach or healthcare liaison, business development roles is highly preferred. Skills : * Strong communication, interpersonal, and presentation skills. * Ability to work independently, as well as part of a team. * Excellent organizational and time-management skills. * Knowledge of hospice care, palliative care, and end-of-life care principles is a plus. Certifications : * Valid driver's license and reliable transportation. * Ability to travel locally for community outreach and healthcare provider visits.
