Job description Summary: To coordinate hospice admissions based on the criteria set out by Hospice of Hope. To review patient referrals, discuss hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care. Qualifications: * Bachelor's degree in Marketing, Business-Administration, or one year as Hospice Liaison. * At least three years' experience in hospice care marketing management. * Ability to market aggressively and deal tactfully with customers and the community through direct market or by ways of community activities. * Have good communications skills, negotiation skills, and public relations skills. * Demonstrate knowledge of Hospice admit guidelines per Medicare and Medicaid guidelines. Job Description: * Assist with Explanation of Hospice Benefits to patients and families. * Call on Physicians, Hospitals, Nursing Facilities, Assisted Living Facilities, and other potential referral sources to develop new business relationships. * Continuously develops a comprehensive marketing plan designed to meet budgetary volume projections alongside the Administrator. * Willingness to conduct in-services in community facilities on Bridge Program and Hospice services. * Maintains positive working relationships with current and potential referral and Bridge candidates.
