DALLAS Hospice Liaison Job Description Position: Hospice Liaison Location: Dallas Job Type: Full-Time Job Summary: The Hospice Liaison serves as a key representative of the hospice organization, acting as a bridge between healthcare providers, patients, families, and the hospice team. This role focuses on building and maintaining relationships with hospitals, skilled nursing facilities, physicians, and other referral sources to ensure appropriate and timely hospice referrals. The liaison educates healthcare professionals and families about hospice services, facilitates smooth patient transitions, and collaborates with interdisciplinary teams to support end-of-life care. Key Responsibilities: 1. Relationship Development & Outreach * Establish and maintain strong relationships with hospitals, skilled nursing facilities, physicians, case managers, and other healthcare professionals to generate hospice referrals. * Educate referral sources on hospice eligibility criteria, benefits, and services through presentations, meetings, and in-services. * Act as a resource and consultant to providers regarding hospice appropriateness and end-of-life care planning. * Participate in community events, health fairs, and professional networking opportunities to increase hospice awareness. 2. Patient & Family Education * Meet with patients and families to explain hospice services, eligibility, and the benefits of palliative care. * Address concerns and provide compassionate guidance in decision-making regarding end-of-life care. * Coordinate with the hospice admission team to ensure a smooth transition for patients choosing hospice services. 3. Referral & Admissions Coordination * Identify and assess potential hospice patients by reviewing referrals, consulting with healthcare providers, and evaluating medical history. * Collaborate with clinical teams to ensure timely admission and proper documentation of patient eligibility. * Work closely with hospice nurses, social workers, and other team members to streamline the admission process. 4. Team Collaboration & Continuous Improvement * Participate in hospice team meetings, strategy sessions, and performance improvement initiatives. * Provide feedback and insights to improve referral processes, patient experiences, and provider relationships. * Stay informed on industry trends, hospice regulations, and best practices to enhance professional expertise. Qualifications: * Education: High School Diploma * Experience: * Minimum 1-3 years of experience in hospice marketing * Experience in case management, discharge planning, or patient advocacy is a plus. * Skills & Competencies: * Strong interpersonal and communication skills with the ability to educate and influence referral sources. * Knowledge of hospice philosophy, Medicare regulations, and end-of-life care principles. * Ability to build trust with patients, families, and healthcare professionals. * Self-motivated with excellent organizational and time-management abilities. * Licensure/Certification: * Licensed healthcare professional (RN, LPN, MSW) preferred but not required.
