Identifies, evaluates, educates, and develops existing and new referral sources in order to generate admissions to HomeCare program and services. Assists the organization in acquisition and retention of customers. Responsible for supporting the patient through the continuum of care by the coordination of services, programs and personnel. Facilitates and ensures the achievement of goals with the use of appropriate resources and multidisciplinary collaboration to provide a coordinated experience for patients and families. * Develop and Grow Your Career! This position is eligible for up to $6,000 per year based upon your Full or Part Time status. * Get Involved! Position: Clinical Liaison HomeCare (Float to all hospitals) Shift: 24 hours per week; shifts will be during Monday through Friday; 8:30 am to 5:00 pm plus 1 weekend per month Experience: 1. RN candidates must have a minimum of three (3) years clinical experience. Home health or hospice experience preferred. 2. Minimum of one (1) year marketing/sales experience preferred. Education: Bachelor's degree in nursing required Licensures/Certifications: 1. Current license to practice nursing in PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
