Full description of the position

This employee operates the day-to-day lead program for the region. * Achieves individual quality and productivity goals in accordance with established objectives * Assists with driving revenue growth across the region JOB FUNCTIONS * Follows up on needs for the patient from Chart note review * Identifies and processes leads across assigned region * Acts as a liaison between hospitals, clinical staff, health plans, providers, and patients to process referrals * Documents referrals, communications, actions, and other data * Trains center staff on leads, managing best practices * Calls MD and Patient to discuss leads and needs * Supports the organization in achieving targets Requirements: Education * High School Diploma or General Education Degree (GED) Required Work Experience * Experience working with homecare patients would be Preferred Knowledge, Skills, and Abilities * Read, write, and perform mathematical calculations and the English language effectively * Work independently * Detail-orientated with excellent organization * Proficient in Microsoft Office programs * Plan, review, supervise, and inspect the work of others
