Nursing & Rehab At Home, a Medicare Home Health Agency serving the San Francisco Bay Area for over 19 years is seeking a Home Health Coordinator (Outside Sales).

A liaison position to ensure the coordination and continuity of health care as a patient transfers from facility to home health.

The Home Health Coordinator is primarily responsible for representing Nursing & Rehab At Home to external customers including patients, physicians, hospitals, skilled nursing facilities, and other health providers. The Home Health Coordinator is responsible for establishing and maintaining working relationships with key stakeholders, generating business from new referral sources and identifying opportunities with key stakeholders, generating business systems/organizations, payors, and the community. The Outside Sales Associate collaborates with the management team, clinicians, and office staff to meet clinical, operational, and financial objectives.

Reports to: Director of Patient Care Services and secondarily to Administrator

Requirements/Qualifications:

• Prior enterprise sales experience 1-5+ years in home care, home health, hospice or related healthcare industry.
• Exceptional knowledge of home care or home health industry preferred.
• Outstanding verbal and written communication skills
• Proof of a valid CA driver's license
• Have reliable transportation & automobile insurance

Job Type: Full-time

Pay: $70,000.00 - $110,000.00 per year
