Full description of the position

As an Home Health Community Liaison you will be responsible for providing high-quality post-acute healthcare services. You will work closely with the Hospital to ensure a more coordinated care experience for the patient.

Responsibilities:

Information Sharing: Share patient clinical and financial information, and other key clinical data to expedite the delivery of coordinated care and allow for monitoring of patient progress. Obtain access to Hospital electronic medical record system (EPIC CareLink) and establish reporting mechanisms for all key metrics.

Provider Network Integration: Partner with selected providers in the Hospital network of post-acute providers, to create optimal communication, hand-offs, and transitions. Develop a standardized handoff process to formally transfer the role and accountability of provider to each next accountable provider.

Facilitate Access to Patients, Medical Directors, and Other Key Clinicians: Accommodate hospital in securing on-site and telephonic access to patients in their facilities/program. Provide hospital with updates regarding a patient’s health and status throughout his or her course of care or service.

Access and Responsiveness: Respond to referrals for admission/care within two (2) hours. Proactively involve patients in Goals of Care discussions, proactively offer Advance Directives and POLST, as appropriate, and provide palliative care services and facilitate hospice services, as appropriate.

Regulatory Compliance: Comply with all applicable laws, rules and regulations of any and all governmental authorities, including HHS, CMS and OIG.

Job Type: Full-time

Pay: $40.00 - $75.00 per hour
