Company Overview:

EasyLiving is a leading provider of Care Managed Home Care Services, committed to delivering exceptional care and support to clients in wherever they call home. We seek a dynamic and motivated Care Navigator to join our team and play a pivotal role in expanding our client base and fostering valuable relationships with referral sources.

Job Summary:

As a Care Navigator at EasyLiving, you will be responsible for proactively identifying and engaging with local influence centers, such as hospitals, nursing homes, assisted living facilities with rehabilitation or respite care, doctors' offices, and other health providers, to educate them about our services and enroll new clients into our programs. Your efforts will contribute significantly to our mission of providing top-notch care and improving the lives of our clients.

Primary Responsibilities:

The Care Navigator will be responsible for, but not limited to, the following:

1. Identify and Cultivate Lead Sources:

  • Research and identify local influence centers that align with our target client base.
  • Build a comprehensive database of potential referrers, including key contact information.

2. Relationship Building and Engagement:

  • Achieve weekly visit frequency with key targets to develop meaningful relationships.
  • Deliver consistent and compelling messaging about the EasyLiving program during visits.
  • Schedule and conduct meetings with individuals in a position to refer clients, including representatives from private, public, and non-profit organizations.

3. Presentations and Networking:

  • Organize and arrange presentations of EasyLiving services at staff meetings in hospitals, nursing homes, doctor's offices, and private companies.
  • Represent EasyLiving at trade shows, conferences, and networking events to build connections and develop additional lead sources.

4. Client Assessment and Care Coordination:

  • Perform client assessments to understand their needs and preferences.
  • Create comprehensive care plans and input them into the CRM program.
  • Coordinate with the office staff to ensure prompt and efficient client care initiation.

5. CRM Management and Reporting:

  • Maintain and manage the Customer Relationship Management (CRM) system to track leads and follow up on potential referrals.
  • Use tracking sheets to record daily activities and submit weekly reports to the manager.

6. Collaboration and Communication:

  • Attend monthly marketing meetings to stay informed about company updates and marketing strategies.
  • Meet with the manager weekly to discuss opportunities, referral status, activity progress, and any challenges.

7. Caregiving Support:

  • Provide caregiving assistance when needed to ensure the continuity of care for clients.
  • Create successful starts of care for newly enrolled clients.

Qualifications and Skills:

  • Bachelor's degree in Marketing, Business Administration, Healthcare Administration, or a related field preferred.
  • Proven experience in sales, marketing, or relationship management within the healthcare industry is a plus.
  • Excellent communication and interpersonal skills to effectively engage with referral sources and clients.
  • Strong organizational abilities with a keen eye for detail and time management.
  • Proficient in using CRM systems and other relevant software tools.
  • Compassionate and empathetic attitude toward clients and their families.
  • Valid driver's license and reliable transportation for travel within the local area.

Pay: $45,312.02 - $50,730.61 per year
