Regional One Health is currently seeking an Inpatient Clinical Rehab Liaison. RRT eligible.

Summary:
The Inpatient Clinical Rehab Liaison serves as liaison to coordinate and link hospital resources, acts as interface with third party payors/home health, coordinates research projects and provides adequate follow-up and rehabilitation for patient; also serves as clinical liaison to identify compliance/quality improvement problems. Provides professional clinical services according to established standards and practices.
Education Requirement:
Graduate of a RN, Rehab (PT, OT, Speech) or Respiratory Therapist (RRT) program required.
Experience Requirement:
Three (3) years of clinical experience in an acute care rehab hospital and/or discharge planning. Demonstrated organizational and communication skills. Experience and skills applicable in area of assignment required.
Licensure/Certification Requirement:
Current licensure in Tennessee as an LPN, RN, Physical Therapist, Occupational Therapist, Speech Pathologist or Respiratory Therapist (RRT), required.
Key Job Responsibilities:
- Attends rounds discussing patient plan with medical staff.
- Facilitates the timely and appropriate completion of pre-admission screening and forms.
- Maintains follow-up and coordinates with patient/family/hospital departments regarding all aspects of patient care needs (i.e. patient transfer, discharge planning).
- Supports and grows the organization by representing and promoting its clinical services, physicians and programs. Serves as the voice of the organization in the medical community and their main goal is to increase patient referrals from existing providers and secure new business from non-referring physicians.
- Addresses patient referrals and authorizations by completing and submitting documents and discussing with providers, insurance carriers and patients. Works under minimal supervision and provides leadership to teams.
- Give presentations on new and existing services. Travel to clinics and hospitals to explain healthcare services and technology to clients.
- Interacts and communicates in a customer responsive and professional manner. Works collaboratively and coordinates activities with hospital/medical staff and community agencies to integrate and implement activities.
- Educate patients about treatment options and answer any questions. Act as a liaison between doctor and patient, collaborating with care team to develop and implement a treatment plan. Provide clinical resources and address barriers to care.
- Document all customer contact and information. Supervise implementation, advocate for the customer's needs, and communicate with client. Identify, prioritize, and reach out to new business categories, partners and opportunities.
Physical Demands/Conditions:
- Frequent walking and standing; lifting and turning patients and equipment; exposure to infectious agents.
