Full description of the position

Skilled Nursing Facility located in Bronx is currently seeking for a Full Time Hospital Liaison.

The Hospital Liaison serves as the main point of contact for a given hospital, account or referral source. The Hospital Liaison will facilitate and coordinate patient referrals and admissions to the organizations various Centers through relationship development with hospital health care professionals- SWs, CMs, DC planners, Physicians, and other medical professionals. The Hospital Liaison will clinically evaluate referrals for potential admission to the Center and assess the patients’ medical record for the purpose of identifying clinical acuity, special needs and projected discharge plan. The Hospital Liaison will provide the appropriate clinical information and financial information to Center internal Admissions Teams who will thoroughly assess clinical and financial criteria prior to determining the referrals acceptance status.

The Hospital Liaison will identify, build and develop synergies within accounts to develop partnerships and collaborations with facilities that will enhance clinical capabilities and patient care and or improve referral volume and build census.

QUALIFICATIONS:

EDUCATION / EXPERIENCE:

· 2 years of experience in an Admissions/Marketing position in healthcare environment

· Long term care experience required

· Knowledge of Medicare and 3rd party billing

· Knowledge of medical terminology

· Ability to discuss clinical evaluations with physicians and other care givers within facility
