Full description of the position

Position Summary:

The Care Transition Liaison will develop, promote and expand the LVMNA Hospice services within our community. The Care Transition Liaison will work closely with the Director of Care Transitions to identify potential new referral sources, develop strategic referral sources within their market area, and continue to cultivate current referral sources through a variety of business

Essential Job Functions:

  • Develop and maintain strategic relationships with hospice referral sources within the medical community.
  • Generate and meet hospice referral expectations by conducting effective and consistent consultative visits with referring accounts.
  • Expand LMVNA’s reach through strategic management of assigned market territory and referral contact database.
  • Increase LMVNA’s market share through proper identification and acquisition of new referral sources. Create and maintain database of referring accounts.
  • Work cross-functionally with the hospice admissions team to meet KPIs for referrals, admissions, and hospice census.
  • Present and deliver information to referral sources on LMVNA’s hospice and end-of-life care services.
  • Meet with hospice patients and family members to discuss LMVNA’s services and explain the hospice benefit.
  • Maintain an in-depth knowledge of Medicare hospice benefit and post-acute care options.
  • Serve as community and educational resource on hospice and end-of-life care.
  • Attend health fairs, networking events and seminars as a LMVNA representative.

Qualifications and Requirements:

  • High school graduate or equivalent, Bachelor’s degree preferred.
  • Minimum two (2) years of hospice field marketing and business development experience.
  • Knowledge of senior post-acute care options.
  • Knowledge of computer, proficient with Microsoft Office Applications
  • Effective communication and interpersonal skills.
  • Strong time management and critical thinking skills.
  • Proven ability to manage market territory and contact relationship.
  • Able to work independently, take ownership and responsibility of tasks.
  • Strong professional ethics, discretion and judgment.
