POSITION DESCRIPTION TITLE: Hospice Physician & Community Liaison

REPORTS TO: Chief Executive Officer FLSA: Exempt

I. PRIMARY FUNCTIONS/GENERAL PURPOSE OF POSITION:

Responsible to the Chief Executive Officer for all aspects of public relations and marketing of the hospice program to the community at large.

II. EMPLOYMENT QUALIFICATIONS:

1. Bachelor’s Degree in business or marketing in a health-related field.

2. At least 5 years experience in hospice, home health or other health-related marketing, or community education.

3. Demonstrated ability in organization, time management.

4. Work requires excellent verbal and written communication skills.

5. Current driver’s license in the state of Virginia.

6. Work requires the employee to travel outside the office and provide their own transportation, with mileage reimbursement from the agency.

III. ESSENTIAL JOB FUNCTIONS:

1. Identifies referral sources in all service areas.

2. Establishes and maintains relationships with primary contact at each referral source.

3. Works directly with the Hospice Medical Director to identify physicians needing education related to hospice services and keeps informed of those needing contact from the Medical Director and/or Hospice CEO.

4. Demonstrates the knowledge and skills necessary to develop a quarterly/annual marketing plan as appropriate that sets out the strategies to meet organizational goals.

5. Meets with the hospice CEO at least quarterly to review marketing plan.

6. Works with staffs of Crater Community Hospice and area nursing homes to increase the visibility, as well as the use of, CCH services in all nursing homes within the service area.

7. Provides Customer Satisfaction Surveys to all accounts at established times.

8. Identifies and participates in community events to increase awareness and visibility of CCH services.

9. Maintains communications with CCH IDT to provide and receive input collected from external customers.

10. Maintains daily communications with the CCH Referral Team to monitor all CCH referral sources.

11. Maintains an accurate and up-to-date customer data base with primary contacts identified.

12. Represents Crater Community Hospice at all times in a professional manner; works within the guidelines of all policies and procedures of Crater Community Hospice, the Virginia Office of Licensure and Certification, and the Centers for Medicare and Medicaid Services (CMS).

IV. OTHER JOB FUNCTIONS:

1. Delivers forms to physicians’ offices for signatures when needed.

2. Maintains statistics as required by the hospice CEO.

3. Prepares reports for the CEO as requested.

4. Performs other duties as requested by the CEO.

V. WORKING CONDITIONS:

1. Works in normal office conditions where there are few physical discomforts due to dust, dirt, noise.

2. Work requires travel outside the agency, approximately within a 50 mile radius. May require provision of service in high crime areas, poor environmental conditions, and non-handicapped accessible sites.

Pay: $65,000.00 - $80,000.00 per year
